JACC: Assistance for Home Care

 

Families often inquire as to what benefit programs are available to provide home care services to their loved one.  Medicaid is one such program that can provide limited home care services to an individual.  An often overlooked program that also provides limited home care services to New Jersey residents is Jersey Assistance for Community Caregiving ("JACC").  JACC, which has somewhat more lenient financial requirements than Medicaid, provides several in-home services that assist an individual at risk of placement in a facility to remain in their community home. This program provides individuals over the age of sixty (60) who are not eligible for Medicaid or Medicaid waiver services. A program care manager helps to assist family caregivers by offering a number of home supports including: respite care, homemaker services, home delivered meals, social adult day care, adult day health, transportation, personal emergency response systems, special medical equipment, chore services, environmental accessibility modifications and caregiver training. A state established sliding scale cost share is required.
